Организации пациентов

Глава, посвященная трудностям продаж, не была бы полной без упоминания пациентских организаций. Они, как правило, финансируются и говорят тем же голосом, что и большая фарма. В 2006 году общеевропейская кампания против рака – Cancer United была представлена в качестве новаторского проекта коалиции врачей, медсестер и пациентов, ратующих за равный доступ к онкологической помощи в Евросоюзе85. Она полностью финансировалась компанией Roche, ведущим в мире дилером лекарств (смотрите главу 2) и производителем препаратов от рака, часть из которых чрезвычайно дороги, например герцептин от рака молочной железы и авастин от рака кишечника. Пиар-компанию проводила Roche, и главное исследование, на котором была основана пропаганда, финансировалось также Roche. Отчет об исследовании был написан Нилсом Вилкингом из Каролинского института в Стокгольме и Бенгтом Йонссоном из Стокгольмской школы экономики. Он получил широкую огласку, но был весьма порочен, и его выводы не были подкреплены данными86. Он заключал: «Очевидно, что в лучших интересах больных раком – чтобы новые, инновационные лекарственные способы лечения становились доступными для них как можно скорее. Снижение доступности или задержка с обеспечением лекарств оказывает очень реальное влияние на выживаемость пациентов».

Это традиционная присказка компаний, и в рекламных материалах говорилось, что кампания нацелена собрать миллион подписей и будет прессинговать Европейскую комиссию на предмет принятия общеевропейской стратегии. Председатель Европейской коалиции раковых пациентов (European Cancer Patients Coalition) оказалась членом исполнительного комитета кампании без ее ведома. Она и члены Европейского парламента отозвали свои кандидатуры из комитета. Председатель исполнительного комитета, профессор Джон Смит, который, будучи редактором «Европейского журнала рака» (European Journal of Cancer), совершил ошибку в отношении одного из исследований маммографии87, написал предисловие к докладу Каролинского института, где признался, что кампания была его идеей, и выразил пожелание, чтобы люди перестали видеть в фармацевтической промышленности врага.

NovoSeven для раненых солдат

В 2011 году компания Novo Nordisk согласилась выплатить 25 миллионов долларов для урегулирования гражданской ответственности в связи с незаконным продвижением препарата от гемофилии – NovoSeven88. Гемофилия – очень редкое заболевание, но компания незаконно продвигала препарат, содержащий фактор VII, специалистам здравоохранения в качестве коагулирующего агента для пациентов с травмами и аналогичными проблемами. Результатом стали ложные заявления при представлении в правительственные программы здравоохранения. Началось судебное разбирательство, и компания подписала «Соглашение о корпоративной этике» по требованию Департамента здравоохранения и социальных служб.

Иск Министерства юстиции утверждает, что Novo Nordisk платила влиятельным врачам Армии США, чтобы они использовали и пропагандировали NovoSeven, а также предоставила исследователям незаконные стимулы89. Ради роста продаж она ввязалась в «мошенническую схему откатов и офф-лейбл продвижения», и в итоге доход за 5 лет увеличился в три раза: до 750 миллионов долларов в 2004 году и в 2007 году превысил 1 миллиард. Незаконная деятельность включала договоренности о выступлениях, продажу мест в консультативных советах и неограниченные исследовательские гранты для людей, работающих в институте хирургических исследований армии США.

В 2005 году в значительной степени искаженное клиническое испытание на пациентах с травмами и тяжелыми кровотечениями (301 человек), пытавшееся доказать эффективность NovoSeven, было опубликовано в малоизвестном журнале90. Если бы это было правдой, эта сенсация скорее была бы освещена в «Медицинском журнале Новой Англии» или в «Ланцете», с огромным тиражом.

Абстракт этой статьи сильно вводил в заблуждение и описывал два испытания, хотя на самом деле оно было только одно. Анализ данных был некачественным: использовался новый исход, не указанный в протоколе, а также произвольное решение о числе переливаний. К тому же были исключены пациенты, погибшие в течение первых 48 часов. Массаж данных был таким неумелым, что становилось очевидно, что клиническое испытание не показало никакого эффекта.

Это испытание был профинансировано Novo Nordisk, и в нем в качестве авторов участвовали сотрудник компании и четыре врача, которым она платила. Эксперты разбили его в пух и прах. В журнале, где оно было опубликовано, говорилось об «информационной прачечной»91, но директор по исследованиям компании Novo Nordisk, Мадс Крогсгаард Томсен, утверждал, что сами врачи настаивали на положительных заключениях, а компания внесла в статью ограниченный вклад92. В 2006 году пять врачей из FDA сообщили, что с NovoSeven были связаны 185 тромбоэмболических событий89. В апреле 2011 года два крупных исследования сделали вывод, что нет никаких доказательств, что лекарство продлевает жизнь по какому-либо из его офф-лейбл показаний, а в исследованиях при инсультах и операциях на сердце NovoSeven по факту повышал риск инсульта и сердечных приступов.

Более всего Сидни Вулфа из организации «Гражданин общества» раздражало, что компания Novo Nordisk годами заставляла врачей одобрять офф-лейбл использование NovoSeven, а потом издала предупреждение о том, что препарат может вызвать потенциально смертельные тромбы у пациентов без гемофилии. Novo Nordisk продвигала NovoSeven среди солдат с 2005 по 2007 год, проводя конференции и семинары, которые имели такие названия, как «Остановите кровотечение! Novo Nordisk отрицала нарушения89, и в интервью датскому радио в 2008 году Мадс Крогсгаард Томсен заявил, что эксперты знают, что препарат эффективен, даже если это не задокументировано научно, и что это объясняет его широкое использование94.
